The Social Worker & Admissions Coordinator is responsible for identifying and monitoring the psychosocial, mental, and emotional needs of the Health Center Members. The social worker will contribute to the care planning process to ensure these needs are met and maintained on an individual basis in accordance with Health Center policies and procedures and current Federal and State guidelines and regulations. The social worker will advocate for an environment that upholds Resident Rights and safely promotes the values of Member empowerment and self-determination. The position is also responsible for coordinating the admissions, transfers, and discharges of Health Center Members, as well as providing education and marketing to Independent Living Members.
Bachelor's in Social Work required.
One year of experience in a skilled setting preferred.

Part Time and PRN opportunities: The Health Center Wait Staff is responsible for preparing and serving therapeutic diet trays to health center Members in accordance with posted therapeutic diet menus. Prepare therapeutic food items as required. Serve meals in the health center in compliance with Member preferences and diet orders. Serve room delivery trays. Monitor acceptance of meals and offer substitute food items for those not well consumed. Assist with tray set-up for Members during all meals. Prepare both health center dining rooms for service, and break down after meals for 3 meals a day. Wash all dishes and pots after each meal and as necessary throughout the shift. Stock items in kitchen, dining rooms, nourishment room, and break room as necessary. Distribute and assist Members in the completion of weekly menus. Complete daily menus for kitchen use in a thorough, timely and accurate manner. Other duties as assigned by supervisor.
